Please fill out this form to request an appointment with me! I only book one month at a time. This form is for January 2026 books at Heritage Tattoo Collective in Frederick, Maryland.
If you are inquiring about multiple ideas or a session with more than one person please only fill out ONE form. 

- I charge by the size, detail & placement of the tattoo. My rate is based on a $250/hr rate for small projects, $200/hr for large scale projects & my minimum is $150.

Pet portraits begin at $500. 

If your request is accepted you will receive a price estimate along with the link to book your appointment.

All booking is done through Calendly.com & all deposits are sent through PayPal, through Calendly.

You must leave a $200 non-refundable deposit to book your appointment. This will go towards the final cost of your tattoo.

- I do not tattoo anyone under the age of 18.
- I am not currently doing cover ups or any color projects at all.
- I currently work Tuesday-Friday no exceptions.

- Please be descriptive when describing what tattoo you want, just putting "tattoo" or "sleeve" will not get a response. Do not write "refer to email/dm" please elaborate so I can easily reference it from here. 
-If inquiring about a pet portrait, PLEASE leave clear reference photos of your pet that you would like to use. Submissions without a photo of your pet will not be considered. 

- Filling out a form does not guarantee that you will be able to book with me. I take on one client a day & work 4 to 5 days a week. There are only so many dates open at a time.

- These applications are reviewed and responded to after my forms close. If your request is accepted, you will receive an email response on or around the 15th or 16th from my email artworkbygia@gmail.com. This email will include your price quote & a link to book your appointment. If your form was denied, do not take it personal, it just means that I am unable to book your appointment at this time or I feel like another artist is better suited for your piece.

-Please be aware of scam emails or scam accounts on TikTok & Instagram. My only email is ArtworkbyGia@Gmail.com & my only social media accounts are under @ArtworkbyG.

Look out for extra letters in the emails or accounts. I only book appointments through these request forms & via email. All booking is done through Calendly.com & deposits are sent through PayPal, through Calendly.com.

-I work Tuesday-Friday, no Saturdays or Sundays.

- Drawings are made the WEEK OF or DAY OF the appointment depending on the piece. A drawing will not be sent before then, and definitely not before a deposit is made. I always leave time in the beginning of appointments to make any and all changes.

- I will NOT copy another artist's work. Every piece I do is custom and inspired by the references and ideas that you send in.
